While PD Adventure Sports Ltd - trading as the Canoe Centre will take all reasonable steps to ensure my safety, I acknowledge that they cannot be held liable for my own actions, for which I must take personal responsibility, or for the actions of a third party. By attending one of our events or activities, I consent to the possibility of filming, photography, and/or live streaming. I also grant PD Adventure Sports Ltd - trading as the Canoe Centre the rights to reproduce and publish my words and/or likeness in any transcript or publication related to the event. If I do not wish to be included in any photography, filming, or streaming, I will notify PD Adventure Sports - trading as the Canoe Centre in advance by emailing activities@canoecentre.ie.
I confirm that I do not have any medical conditions or illnesses other than those disclosed on this medical consent form. I declare that I am sufficiently fit, able, and competent to take part in the activities as specified in the booking.
